The NHSBSA issues these for claiming free treatment you weren't entitled to. Many are issued in error — exemption certificates that lapsed during treatment, or admin mismatches.
Common grounds
- You were entitled at the time — certificate or benefits valid on the date of treatment
- You renewed your exemption certificate but the renewal hadn't been processed
- You ticked the wrong box by mistake — first offence
- Penalty was sent to the wrong person

How much can the penalty be?
Up to 5× the original charge plus a £50 surcharge. Caps at £100 for first offences in some cases.
What if I genuinely couldn't pay at the time?
Low income / NHS Low Income Scheme entitlement at the date of treatment is a complete defence — get a HC2/HC3 certificate.
